Phyllis Ann Arnett Kozlok Sampson

Phyllis Ann Kozlok Sampson passed away on Saturday, April 28, 2018 at the age of 86. Phyllis unexpectedly but peacefully died in her own home with her loving and cuddly companion, Olivia, nearby. Phyllis was born on June 9, 1931 in Columbus, Ohio, the daughter of the late Ralph and Annabelle (Yontz) Arnett. She attended Reynoldsburg High School and then met her first husband, the late Frank Kozlok, in Port Clinton. They raised two children together. Phyllis worked as a customer service representative at Ohio Edison until her retirement in 1982. She then married the late William Bud Sampson and resided in Columbus, OH. After his death, Phyllis then returned to Port Clinton in 2001. Phyllis was active during her long periods of time in Port Clinton. She was a Den Mother for the Boy Scouts, a choir director and Sunday school instructor for Peace Lutheran Church. Phyllis was an athlete and was well known as a terrific softball pitcher, a volleyball spiker, a 600 Club level bowler, and a wonderful line dancer. She loved poker, bingo, and was an I-Pad solitaire addict. Phyllis also loved shopping in her many catalogs. Survivors include her younger sister, Nancy Lewis, Vine Grove, KY; her son, Mark (Mary Mattoon) Kozlok, Port Clinton; daughter, Melody (Julie Hermes) Kozlok, Cincinnatti, Ohio; grandchildren, Amanda (Delbert) Hatt and Stephen (Cindy) Kozlok; great-grandchildren, Lilly and Chloe Hatt and Summer and Autumn Estevanes; sister-in-law, Wilma Arnett, Columbus, Ohio; as well as nieces and nephews. She was preceded in death by her parents, her husbands, sisters, Dorothy Ballenger, Barbara McConnaughy; and brother, Ralph Arnett. Visitation will be held on Tuesday, May 1, 2018 from 6-8 pm at the Neidecker, LeVeck & Crosser Funeral Home, 1124 Fulton Street, Port Clinton. Additional visitation will be held on Wednesday, May 2, 2018 from 11 am until Funeral Services at 12 pm at the funeral home with Pastor Dane Meuschke officiating. A reception will be held immediately following the service at Peace Lutheran Church, 900 Jefferson Street, Port Clinton. Burial will take place on Thursday, May 3, 2018 at 1 pm at Carroll Cemetery in Carroll, Ohio Memorial contributions may be made to Peace Lutheran Church.
