Phyllis A. Cook

Phyllis A. Cook, 73, of Port Clinton, Ohio died Tuesday, March 2, 2010 at the Cleveland Clinic, Cleveland, Ohio. She was born on January 19, 1937 in Newburg, West Virginia, the daughter of the late Lester P. and Viola M. (Bell) Bolyard. She married Howard Cook on March 23, 1956 and he survives. Phyllis worked at the U.S. Gypsum Company in Gypsum, Ohio until her retirement in 1999. She attended the Church of God of Prophecy in Port Clinton. Her hobbies included cooking, crocheting, working on crossword puzzles, and collecting windchimes and Elvis plates. She especially enjoyed her kids and grandkids. Survivors include her husband: Howard; sons: Ronald L. (Cindy) Cook, Oak Harbor, OH; Terry L. (Marie) Cook, Port Clinton, OH; Brian H. (Sonya) Cook, Custar, OH; grandchildren: Jimmy Cook, Tammy (Lee) Dewey, Graham Deak, Betsy Cook, Benjamin Cook, Brice Cook, Ashley Cook; great grandchildren: Skyler Cook, Nick Dewey, Madison Dewey, Kennedy Deak, London Deak, Conner Ambrozy; sisters: Dorla (Bill) Moritz, Port Clinton, OH; Mary (Earl) Taylor, Oak Harbor, OH; Joann (Ted) Denslow, Oak Harbor, OH; brothers: Paul (Brenda) Bolyard, Oak Harbor, OH; Donald (Peggy) Bolyard, Castalia, OH; Roger (Bonnie) Bolyard, Port Clinton, OH; Jason (Becky) Bolyard, Oberlin, OH; Ralph Bolyard, Bay View, OH; and her beloved dogs: Shep and Jake. She was preceded in death by her parents, grandson: Kylee Cook, and infant sister: Wilma Mae Bolyard. Visitation will be held on Friday, March 5, 2010 from 2-4 and 6-8 PM at the Neidecker, LeVeck & Crosser Funeral Home, 1124 Fulton Street, Port Clinton, Ohio. Additional visitation will be held on Saturday, March 6, 2010 from 11:00 AM until funeral services at 12:00 PM at the funeral home with Reverend Earl Taylor officiating. Burial will follow at Christy Chapel Cemetery, Portage Township. In lieu of flowers, memorial contributions may be made to the Shriners of Toledo.
