Addison Anne Jadwisiak, died shortly after birth on September 2, 2008 at the Toledo Hospital in Toledo. She was the daughter of Robert E. and Jaime C. (Adkins) Jadwisiak of Port Clinton. In addition to her parents, she is survived by her brother Nicholas and her grandparents, great grandparents, uncles, aunts and cousins. Visitation will be held from 10 a.m. Monday until the time of services at 11 a.m. Monday at the funeral home. Burial will follow in Catawba Island Cemetery.
